Triage Practitioner (Social Worker)

Working directly with whanau of Visionwest Community Trust, you will be responsible for the coordination of client, tenant and provider referrals or enquiries. The role would provide a first point of contact and triage service for clients and providers to ensure that they are directed to the most appropriate team members to meet their needs. 

You will also:

  • Support whanau by providing individual assessments and plans, connecting them with life skill services, advocacy and supports
  • Provide an integrated and holistic approach to care for whanau
  • Hold a client caseload based on capacity & workflows

You will need:  

  • A qualification or registration with New Zealand's Social Worker Registration Board in Social Work with at least 3-5 years’ experience 
  • Administration, Data input, handling, and analysis skills
  • Computer literacy
  • Professional and personable phone and face-to-face interaction skills
  • High level of accuracy and attention to detail
